Onsite and Telephonic Assessments: CarePraxis offers specialty assessment tools for obtaining current health status and care information:

Basic Onsite Assessment (BOA)

Expanded Onsite Assessment (EOA)

Onsite Care Coordination Assessment (OCCA)

By contracting with licensed healthcare professionals across the United States , CarePraxis:

  • assesses an individual’s abilities to perform daily self-care activities,
  • reviews safety concerns, including cognitive and environmental factors,
  • evaluates current care resources and assistance needed,
  • reviews health status and care being provided by family and/or paid caregivers, and
  • reports any changes in care needs and resources during a specific timeframe.

Individualized Care Plan: Our experienced professional registered nurses develop individualized care plans based on information obtained during assessments. Local community resources are considered in addition to family availability. Care needs are viewed in a holistic framework.

Onsite and Telephonic Medical Record/Physician Statement Retrieval: When a specific medical record or physician statement is needed, CarePraxis will call and/or fax a request, then follow up to retrieve the items by fax, mail, or in person.

Care Resources Survey: Care resources, services and rates are shopped and compared in a specific geographical area in an effort to locate the best fit with an individual’s financial resources, including insurance benefits, and a list is provided for personal selection.

Care Transition Assistance: When the need arises for discharge from a facility or for transition to another care provider, CarePraxis’ RNs will assist with initiation of care, transition to a different agency’s care, and/or to family assistance and self-care.

Initial Care Coordination: CarePraxis’ professional registered nurses make telephonic contact with an individual, arrange for an objective Onsite Care Coordination Assessment (OCCA), dialogue with the selected agency about care needs, review the initial physician’s plan of care, and develop and submit a report describing the functional/cognitive deficits that affect ongoing care. The Initial Care Coordination Report (ICCR) provides a snapshot of the potential impact of care needs and costs on financial resources, including insurance benefits. Upon receipt of the OCCA, an Onsite Care Coordination Assessment Summary (OCCAS) will also be prepared and submitted.

Care Coordination Follow Up: Continued telephonic contact with an individual about their functional/cognitive status and the effectiveness of care being provided will be reported by CarePraxis RNs at appropriate time intervals based on the anticipated length of care.

Telephonic Interview: In some cases, additional information about an individual’s care needs or care being provided can be important. CarePraxis will make telephonic contact with a source to gather data or update useful information.

Home Modification Preview/Review: CarePraxis contracts with rehabilitation therapy professionals for onsite previews with photos of existing structural and environmental features that may be remodeled or refurbished to best meet the care needs of an individual. After construction is complete, the therapist will review and photograph the changes and report the outcome of such home modifications.

Level of Care/Schedule Verification: A scheduled or unannounced visit may be made by CarePraxis during a specified timeframe to verify that the reported caregiver schedule is worked and/or that the reported level of care is received by an individual in a facility or at home.

Provider Survey: An in-depth survey of the licensure, care services, staffing ratio, specialty training, billing records, and other information for a facility, agency, or physician office may be obtained by CarePraxis.

Caregiver Skills Interview: CarePraxis’ licensed healthcare professionals can interview privately-hired caregivers and document skills and experience stated/observed during a face to face interview.

Special Questions: For situations requiring additional information not included in assessments or other services, such as Restoration of Benefits, CarePraxis will devise a special questionnaire to be used by assessors to obtain necessary answers.

Residence Verification: For an individual reported to reside in a care facility, CarePraxis can assist in verifying that care continues to be needed and provided at the specified facility.

Other Onsite Visit: Licensed healthcare professionals with areas of expertise are available for onsite services other than those listed here. CarePraxis will assist in the development of reporting tools to gather necessary data.

Individual Record Review/Nursing Summary: CarePraxis’ experienced auditors will develop audit tools, review and contrast available records, trend findings, and provide a nursing summary of results.

Anti-fraud Training: Educational programs on avoiding and identifying potential fraudulent activity are available through CarePraxis. The background of the company officers in nursing, elder care, long-term care insurance services, claim investigations, patient advocacy, fiscal monitoring, and management brings a unique viewpoint to fraud prevention and detection.
