  Does CarePraxis provide care planning? Yes, CarePraxis assists the individual (patient) and family in developing plans for care needs.

Will CarePraxis discuss my condition or care with others? Information about you will only be disclosed by CarePraxis as authorized by you and as required by law to persons or organizations that have a need to know about your situation.

Do I have to be receiving care or needing care now to use CarePraxis? CarePraxis can assist with proactive steps to plan for future care needs, as well as current ones.

Will CarePraxis provide the actual care? CarePraxis is a provider of information, not direct care.

Does CarePraxis provide care coordination? CarePraxis coordinates care through communication with the individual (patient), family, physician and other care providers to ensure that care is effectively meeting needs and expectations.

What does care coordination include? For care coordination, CarePraxis first assesses the overall care needs (medical concerns, safety and home environment, physical assistance and equipment needs, personal resources and family availability, and community service resources) to develop a plan of care that best fits the desires of the individual (patient) and family and can be communicated with the physician and other care providers.

Why does CarePraxis interact with the physician and caregivers? CarePraxis coordinates communication with the entire care team, including the individual (patient), family, physician, paid and voluntary caregivers, and payers, to ensure quality of care.

How can care coordination help me and my family? Care coordination can deliver peace of mind for the individual (patient) and family that all care needs are being addressed and that care is being monitored by professionals with expertise in eldercare.

How does CarePraxis monitor care? CarePraxis monitors care through telephonic and onsite interactions with the individual (patient), caregivers, and other sources initially and at specified time intervals for review of changes in care needs and effectiveness of care provision.

Is a nursing home the only care option? Regardless of the care setting (your home, a retirement community, an assisted living situation, or a nursing facility), CarePraxis will monitor ongoing care needs and whether care being provided is adequate in meeting care goals.
