Privacy Policy

CarePraxis appreciates that the privacy of information is a matter of trust with its clients, so the following privacy policy has been established and will be maintained.

CarePraxis maintains security policies and practices in its offices and systems to ensure appropriate protection of information from unauthorized access and inappropriate disclosure, alteration or destruction. Security may include, but is not limited to, physical access safeguards, electronic encryption and other technologies. CarePraxis periodically reviews and enhances security systems as necessary. Policies and procedures are also in place to facilitate training and monitoring of confidentiality of private information. CarePraxis follows laws and regulations governing modification of medical information obtained from other sources. CarePraxis does not provide private information to the general public, nor does it distribute mailing lists with client information obtained from written or electronic data, i.e. “cookies” stored on a user’s Web browser. As CarePraxis upgrades, enhances, modifies or maintains its systems, services and procedures, modifications may be made to this policy.

CarePraxis respects the privacy of individuals while obtaining and utilizing information for legitimate business purposes. CarePraxis obtains and discloses such information pursuant to appropriate written authorizations to gather information for, and disclose it to, its clients for the purposes of assisting with planning, monitoring, and coordinating care, and for their other legitimate purposes authorized by law. CarePraxis handles such information in accordance with its clients' requests, the written authorizations, and applicable law. CarePraxis does not share information obtained for a client with its other clients, unless the subject's authorization permits such.

CarePraxis is authorized by clients to obtain and use information about demographics, care providers, medical information, healthcare needs, family availability, living environment, physical capabilities, cognitive testing, insurance coverage and other resources related to healthcare. Sources of such information may include various governmental agencies, physicians, medical agencies or facilities, community organizations or individuals involved in care.

CarePraxis does not use the information it obtains except as instructed and authorized by its clients. CarePraxis follows applicable privacy laws in regard to rescinding an authorization to release information (explanation of that process is stated on CarePraxis’ authorization forms).
